Date of admission- 12/09/2021 A 70-year-old farmer by occupation came, with a chief complaint of stomach ache for 15 days, pedal oedema (on and off ), shortness of breath on exertion since 2 months, loose stools 4-5 episodes around three days back and got subsided, with decreased urine output for 3 days, high-grade fever since 4 days, got receded, drowsiness since 2 days.   A 60 year old man, carpenter by occupation came to the casuality with chief complaints of  Loss of speech since morning  HISTORY OF PRESENT ILLNESS :  Patient was apparently asymptomatic 3 years ago  3 years back, where he attended a family function and had an binge of alcohol consumption, after a few hrs of consumption, he suddenly developed deviation of angle of mouth towards left side, for which the attenders took him to a local hospital .He was treated conservatively (? Medication unknown) and adviced to use those medications for 1 week.After a week ,his deviation of mouth was resolved.There in the hospital he was diagnosed of having a renal problem ,as B/L pedal edma(pitting type) and facial puffiness was found to be present on examination.Even his creatinine & blood urea was found to be elevated .He was given lasixs and sodium bicarbonate (OD).  GM Case history -2  August 23rd Date of admission  - 22-01- 2021  A 22y- old - woman  presented with  blurry vision .   History of Present illness  The patient was apparently asymptomatic  8months back ,then she had blurry vision for a week  and Hypertensive ( BP 220/70mm Hg)  .Consulted to an Opthomologist , found a clot in the eye , suggested a complete body scan .  History of Past illnes -  No history of Weight loss. Not a known case of HTN, CAD, Asthma, TB, Epilepsy, Thyroid disorders. No history of surgeries and blood transfusions in the past.  Personal History  No history of consumption of alcohol. History of low /minimum intake of water . FAMILY HISTORY - No history of DM, hypertension, CVA, CAD, Asthma, Renal disorders. Thyroid disorders in the family.
