Date of admission - February 3  43 year old female patient presented to the opd with chief complaints of low backache since 2 days, vomitings and pain in the abdomen since 1 day.  History of present illness- Patient was apparently asymptomatic since 6 years back , then she had anasarca for which she was admitted in hyderabad. There she was diagnosed with hypothyroidism ( tab. Thyronorm 50 micrograms  ),renal failure (tab. Chief complaint:  A 19 year old male student came to casualty with chief complaints of abdominal pain , vomiting episodes  (2 -3) since 3 days , loose stools  since 3 days.   History of present illness :  Patient was apparently asymptomatic 5 days ago. Date of Admission : 19-12-2021 A 48-YEAR-OLD MAN WITH GRADE IV SOB. A 48 year-old man came to the hospital with chief complaints of p edal edema since 1 month,s hortness of breath which aggravated since 2 hours,p roductive cough since 2-3 days.  History of present illness : The patient was apparently asymptomatic 1 month ago when he noticed swelling in both his legs which was gradually progressive, extending to his knees.   H istory of   increased shortness of breath since 1 day which aggravated to Grade 4 SOB since 2 hours, associated with orthopnea and PND.  H/o Productive cough since 2-3 days.
